What to bring

When visiting Wollongong Eye Specialists please ensure you bring the following with you:

Patient Information form
New Patients must bring their referral letter from their GP or Optometrists with them to their initial appointment
Medicare Card
Glasses (if you wear them)
List of medications you currently are prescribed
Someone to drive you home after the appointment because patient's eyes may be dilated.
